Dear Editor Frohlich and Dobiasova (1) measured the plasma levels of triglyceride (TG), total cholesterol (TC), apoB, low density lipoprotein cholesterol (LDL-C), and high density lipoprotein cholesterol (HDL-C), and calculated the Log (TG/HDL-C) and the fractional esterification rate in apoB-depleted plasma (FERHDL) to evaluate the relationship between these data and angiographically-defined coronary artery disease (aCAD). They concluded that FERHDL is the best laboratory predictor of the presence of coronary atherosclerotic lesions. LDL B (small dense LDL) is a risk factor for coronary atherosclerotic lesions (2). We are interested in the relationship between the parameters they investigated and LDL B. We fractionated lipoprotein into LDL A, LDL B (s, d LDL), HDL3, and HDL2 by 2-15% polyacrylamide gradient gel electrophoresis. The gel was immersed in buffer for electrophoresis, and incubated at -20°C for 2 hours, facilitating direct staining of TG and TC in each fraction. Sudan Black (SB) was used for staining of total lipids in each fraction, and a staining kit (Helena Inc.) was used for staining of TG and TC. When the electrophoresis pattern was compared between healthy adults and patients with aCAD, the level of HDL3 was decreased in the patients with aCAD in whom LDL B was detected, whereas the level of HDL2 was increased Lecithin-cholesterol-acyltransferase (LCAT) contributes to changes from HDL3 to HDL2. An increase in FERHDL is also closely related to LCAT activity; our results were consistent with the findings reported by Frohlich and Dobiasova (1). However, to evaluate the risk of arteriosclerosis, the lipid composition of lipoprotein should be comprehensively evaluated; our differential staining technique of lipoprotein fractions, such as TG and TC components, by freezing may be important.
